COVID-19 Office Procedures

Dear Friends,
We hope you are staying healthy, brushing twice a day, and enjoying the crisp fall days.  We are continuing to monitor this Covid-19 situation and are prepared to make some changes to our arrival protocol. Maintaining your health and safety is of utmost importance to us.  

First, we are still requiring a COVID-19 Disclosure form to be completed within 24 hours of your appointment.  However, if you have been fully vaccinated you will see that the form has been abbreviated.

 On our website, www.Drmatthews.com, you will also find your Medical History, Sleep Questionnaire and Dental Insurance Information under the “Patient Forms” tabs.  Please fill out ALL PATIENT FORMS and hit submit.

If you cannot fill out these forms online, please arrive 10 minutes prior to your appointment time so that we can give you copies of the forms when you arrive.

 Second, the Delaware State of Emergency has been lifted. Please feel free to enter the office without calling to announce your arrival.  Since we are classified as a medical facility, we still need you to wear your mask upon entering the office. We will continue to monitor your temperature and you will be escorted to your treatment room.
